Our hours of operation are from 8:00am-4:00pm. The doors to the ATRB building will lock at promptly at 4:00pm so try to arrive earlier. We are also proud to present our new submission form layout. We listened to our client's feedback providing more space to describe the problem on the front page by moving the table with questions on the back page.
When looking for our form, please make sure to visit our website (clinic.ipm.iastate.edu), which will link to the extension store, the permanent location of the submission form.
Our form is a living document that can be filled by typing on a pdf reader program, printed and included with your sample. Alternatively you can print the form and hand write the information.
We are transitioning to a new website clinic.ipm.iastate.edu where we have added more photos and videos!. All of our contact information and form can be found at our Services and Information page. Explore our pages by service: Insect ID, Mushroom ID, Plant/weed ID, Nematode Testing, Plant Problem (scroll to see Specific Plant Submission Instructions -click on the plant type name).
